The character of the fluid of each vas deferens is evaluated at the time the vasectomy reversal. The quality of the fluid coming from the testicle side of the vasectomy site is evaluated for its visual appearance and the microscopic findings.
The visual appearance:
The less creamy the fluid appears-usually the better the microscopic findings. Clear is better than milky, milky is better than creamy, and creamy is better than paste.
The microscopic findings:
Over time after a vasectomy, the body slowly begins to breakdown (absorbs) the sperm. The microscopic presence of whole sperm is most the favorable finding for a successful pregnancy. Increasing degrees of breakdown is associated with decreasing rates of success.
The number of years since the vasectomy influences the quality of the fluid:
In short, the less creamy the fluid appears on visual exam and the less break down of sperm noted microscopically-the better the success rate of the vasectomy reversal.
In general, the quality of the fluid (both visually and microscopically) is negatively affected by the increasing interval since the vasectomy.

Why is the quality of the fluid a predictive factor for vasectomy reversal success?
Because the vasectomy site is transected, fluid is released from the testicular side of the vas tube. This fluid is examined at the time of the reversal surgery. As a result, this helps predict the percentage of success.
The character of the fluid of each vas deferens is evaluated at the time the vasectomy reversal. The quality of the fluid coming from the testicle side of the vasectomy site is evaluated for its visual appearance and the microscopic findings.
The visual appearance:
The less creamy the fluid appears-usually the better the microscopic findings. Clear is better than milky, milky is better than creamy, and creamy is better than paste.
The microscopic findings:
Over time after a vasectomy, the body slowly begins to breakdown (absorbs) the sperm. The microscopic presence of whole sperm is most the favorable finding for a successful pregnancy. Increasing degrees of breakdown is associated with decreasing rates of success.
The number of years since the vasectomy influences the quality of the fluid:
In short, the less creamy the fluid appears on visual exam and the less break down of sperm noted microscopically-the better the success rate of the vasectomy reversal.
In general, the quality of the fluid (both visually and microscopically) is negatively affected by the increasing interval since the vasectomy.

Why Is That?

Why is the skill in using an operating microscope and having an experienced surgeon reverse your vasectomy important?
Interestingly, the answer is complicated. Below detail several of the reasons. Notably, only 5% of the men who have had a vasectomy will desire to reverse it. As a result, this limits the number of patients who need a reversal. Furthermore, most urologists uncommonly see patients who need a reversal.
Additionally, insurance does not cover a vasectomy reversal. The surgeon offers a package plan for all of the costs involved. In other words, the surgeon coordinates, and pays the various entities necessay to perform a reversal.
Incidentally, Dr. McHugh utilizes an operating microscope and uses microscopic suture to join the very small openings. Additionally, he places 14-16 sutures in each vas deferens.
For example, the thickness of a strand of hair is 100 microns.
Moreover, the surgeon operates with suture measuring 70 – 100 microns. Therefore, the suture is about the diameter of the human hair.
Of note, the outside thickness of the vas deferens is about 2 mm. The lumen of the vas (the inner channel) averages about 5.6mm in diameter.
In addition, 500 microns is about the size of the opening in the vas deferens (.5mm).
As a matter of fact, the lumen of the vas deferens is approximately the size of the โOโ in God on a penny.
Interestingly, Dr. McHugh places 14-16 microscopic sutures the size of hair to join a tube that is less than a millimeter in diameter.

Using an operating microscope, suture the size of hair (9-0 Nylon) is used to align the openings of the vas tubes. Dr. McHugh uses the Modified Two-Layer technique. Therefore, the first layer joins both the outside of the vas and the inner opening (the channel that carries the sperm).
The second layer joins the outside of each of the vas tubes. As a result, this method provides for both the proper alignment of the lumen as well as support of the repair assuring a tension free connection.
Studies have shown that this technique has similar success rates as other methods which take longer to perform and can incur a greater expense. A total of 14-16 microscopic sutures are placed on each side.Of note, the procedure takes two hours to perform. In summary, the entire time spent at our facility is usually around 3 hours.

Dr McHugh and his staff did an amazing job with me and my wife during my reversal. They treated us so well from the beginning to the end. We went to Dr. McHugh because he was the most experienced and happens to be 5 hours away. Iโm day 4 right now and am doing well. I will update if the surgery was successful. If the Lord allows we may have another baby. Also, I love the fact that if I have questions I can ask the Dr himself or his lovely staff.

Three factors are very important and as a result, impact on the success of a vasectomy reversal:
The number of years since the vasectomy.
The skill and experience of the surgeon.
The findings at the time of surgery (the quality of the fluid).

Using an operating microscope, suture the size of hair (9-0 Nylon) is used to align the openings of the vas tubes. Dr. McHugh uses the Modified Two-Layer technique. Therefore, the first layer joins both the outside of the vas and the inner opening (the channel that carries the sperm).
The second layer joins the outside of each of the vas tubes. As a result, this method provides for both the proper alignment of the lumen as well as support of the repair assuring a tension free connection.
Studies have shown that this technique has similar success rates as other methods which take longer to perform and can incur a greater expense. A total of 14-16 microscopic sutures are placed on each side.Of note, the procedure takes two hours to perform. In summary, the entire time spent at our facility is usually around 3 hours.

The vast majority of our reversal patients live outside of the Gainesville area. For this reason, a hotel stay is provided for patients preferring an overnight stay. In addition, we include this expense in our package price.
